- Design your custom header .
- Submit the header in a Word document, formatted exactly as you want it to appear on the standard reports.
- Header must remain at size 0.3”.
- Header must be sent to eIVF in a table format on a Microsoft Word Document.
- For logos, please provide a high-resolution image(300 dpi) in .png format with a transparent background.
- Resize the logo to the desired size and cut and paste into the table format along with other header information. When resizing the logo keep in mind the additional information that will be included i.e.; address(s), phone numbers, physician names, and web address.
- The logo along with other additional information must be added to the table.
- Logo must have a transparent (checkerboard) background to accommodate resizing of the image. If the image is not transparent we lose the crisp look of the image when resizing it to fit on the internal documents.
- Important: Double-check all spelling, numbers, spacing, and formatting before submitting.
